Nova Scotia Health and Cape Breton University have partnered to create a primary health care clinic for newcomers in the Cape Breton Regional Municipality (CBRM). This role is for a full-time limited term position until February 13, 2026.

Cape Breton University is seeking a dedicated Social Worker to support patients and families new to Canada within the past five years. This role involves providing case management and assistance with navigating the healthcare system and accessing community resources.

As a Social Worker reporting to the Director of Health and Counselling, you will be part of a multidisciplinary team, helping clients secure necessary resources for health and wellness. Your responsibilities will include conducting comprehensive assessments, liaising with community agencies and organizations, and developing and delivering educational sessions for individuals, families, and community groups.

The ideal candidate will hold a Bachelor of Social Work (BSW) and be registered with the Nova Scotia College of Social Workers. A criminal background check and sex offence registry check are required. You should possess excellent knowledge of community services available to newcomers, demonstrated ability to work collaboratively in an interdisciplinary team, and experience in assessing and addressing social determinants of health.

You will be expected to plan, develop, and evaluate health programs and care plans, contribute to research initiatives, and assist clients with navigating public and private health insurance. The role also involves maintaining confidentiality, adhering to Nova Scotia’s Personal Health Information Act (PHIA), and providing outreach services to newcomer communities.

The clinic provides timely access to safe and culturally sensitive care for those without a permanent primary care provider (physician or nurse practitioner).
