Description

The Coordinated Access Manager is responsible for the set-up, management, promotion, adoption, and ongoing development of the Coordinated Access in the CBRM (Cape Breton Regional Municipality).

 

Funded by the CBRM Reaching Home Community Entity, Coordinated Access aims to bring together organizations that serve individuals who are homeless or at risk of homelessness to articulate and achieve a set of community objectives that include a reduction in chronic homelessness, a reduction in homelessness overall and for priority populations, reductions in new inflows to homelessness, and a reduction in returns to homelessness.

 

The Coordinated Access Manager serves as the secretariat to the Governance committee and works with the Governance committee and other community stakeholders to examine and accomplish the objectives laid out in the Reaching Home Coordinated Access Guide.

 

Duties and Responsibilities

Governance Committee:

  • Coordinate and regularly convene local governance committee
  • Ensure governance committee includes appropriate representation
  • Recommend and outreach to new potential committee members as needed
  • Support committee in timely decision-making to make to advance CA program
  • Ensure committee has current Terms of Reference (Roles and Responsibilities)
  • Ensure committee has articulated clear coordinated access outcomes, local priority populations, and 12-month governance committee goals
  • Onboard new committee members
  • Record keeping for committee

HIFIS Implementation and Compliance:

  • Ensure implementation and compliance among primary group of users
  • Build understanding and adoption among secondary group of users
  • Create visual materials to aid in broader understanding and adoption
  • Work to build trust in HIFIS and its management and knowledge of its importance
  • Assist in troubleshooting for users

System Map (Creation and Maintenance):

  • Create a system map of services for homeless/at-risk of homelessness populations
  • Set up map as precursor to future Coordinated Access Shared Resources Inventory
  • Consider making map available online/searchable/designed for community users

Data Management, Analysis, and Sharing for Community Planning and Education:

  • Ensure policies and protocols in place and adhered to for data privacy and safety
  • Respond to data breaches and improve system to avoid future breaches
  • If/as needed create user guides to enhance data privacy and safety
  • Identify junctures and ways to analyze and share data with governance committee, other stakeholders, community, municipality, and province in service of elimination of homelessness

Access Points (AP), AP Protocols, Shared Assessment and Triage Tools:

  • Work with local governance committee towards the set-up and promotion (communication throughout community) of CA access points and protocols
  • Work with the committee towards a set of shared assessment and triage tools
  • Work towards a coordinated system that both serves local needs and achieves the full potential of coordinated access in the CBRM.

Qualifications

  • Relevant post-secondary education or equivalent experience
  • Minimum of three (3) years in housing or community dev./community eng. 
  • Experience working with and advocating for vulnerable populations 
  • Ability to work independently, as part of a team, and with guidance
  • Exceptional attention to detail and verbal and written communication skills 
  • Ability to work effectively with others and lead others towards a shared goal 
  • Highly organized and ability to work well under pressure 
  • Proficiency with Microsoft suite of programs and applications 
  • High level of integrity, confidentiality, and accountability 
  • Understands servant leadership and community-centricity
  • Experience using, and ideally managing/manipulating databases
  • Compassion, empathy, diplomacy, and problem solving 

Work Conditions

  • Travel may be required 
  • Manual dexterity required to use desktop computer and peripherals   
  • Overtime may be required; working evenings/weekends as needed 
  • Interacts with employees, management, and the public at large  
  • Environment can be busy and/or noisy at times
